Medical Authorization: I, as a parent or legal guardian, grant Live Love Learn Chinese staff the right to authorize emergency medical treatment for my child in the event that I, or my designated representative, cannot be reached.

Activities Authorization: My child has permission to participate fully in the enrolled Live Love Learn Chinese activities. Activities will occur inside the assigned facility and may include occasional neighborhood walking tours or mini field trips to local stores or restaurants that are relevant to the class content with prior notice.

Liability Waiver: I acknowledge and understand that my child may engage in activities that involve risk of injury, including disability or death and severe social and economic losses which might result not only from my child’s own actions, in-actions, or negligence, but the actions, in-actions, or negligence of others, or the conditions of the premises or of any equipment used, and further, that there may be other unknown risks not reasonably foreseeable at this time. I assume all the foregoing risk and accept personal responsibility for any damages following such injury, permanent disability, or death, and hereby release, discharge, covenant to indemnify and not to sue Live Love Learn Chinese, and their affiliated organizations, their employees, administrators, and associated personnel from any claims or liability by or on behalf of me or my child arising as a result of my child’s participation (including without limitation, personal injury, property damage, or death), either directly or indirectly.

Safety and Participation Policy: I understand that Live Love Learn Chinese is committed to providing a safe, inclusive, and enriching environment for every child, and I agree to support this standard as a condition of my child’s enrollment. I acknowledge that if my child’s behavior poses a safety risk to themselves, other students, or staff - including physical aggression toward others - or if my child is unable to participate in a way that is significantly disruptive to the learning environment despite reasonable support efforts, Live Love Learn Chinese reserves the right to contact me immediately and require same-day pickup of my child. I understand that enrollment fees are non-refundable in such cases. I recognize that Live Love Learn Chinese staff will make reasonable efforts to support all participants, and I agree that the safety and wellbeing of all children in the program is the highest priority.

Photo Consent: Live Love Learn Chinese has my permission to use my child’s image in any form of marketing (social media, brochure, website, television, newspaper, etc.) for the purpose of promotion without listing any personal information (name, age, school, etc.) of my child. I waive all claims for compensation for the use of the photographs and audio content and I hereby release all members of Live Love Learn Chinese, its employees and volunteers from any claim or liability arising from the taking or use of photographs, recordings and interviews.
